The crucial time period for hospital readmission is the first 2-3 weeks following hospital discharge.  While many readmission are due to the original illness, many are due to problems surrounding communications/self-care following discharge.

Poor medication management is a common preventable cause of unplanned medical readmissions in the elderly.  It is also a factor causing falls in the elderly.  Work with your pharmacy.  Most pharmacies can repackage pills that should be taken together in a “calendar card”, an easy blister pack.  This service is usually free.  Medication reminders can be provided by our Caregiver.
Lack of proper diet/fluid intake accounts collectively for a significant number of readmissions to hospital.  We can recommend a home meal provider or our caregiver who can help with meal preparation or special dietary needs for you.

Top Tips for a Safe Transfer Home:

Always follow your Health Professionals advice.
Give your letter to your G.P. or get a family member for caregiver to deliver.
Mark your next appointment date in your diary.  Organise a family member or caregiver to accompany you.
Have your prescription filled and get a blister/calendar pack where possible.
Get a family member or Caregiver to stock up on easily prepared meals.
Ensure your heating is turned on and that you have plenty of fuel.
Drink plenty of fluids.
Don’t overdo it during the first few days home.
Let your neighbours know you are home.
If you are living alone, it is a good idea to have an emergency response pendent.
If you are worried about unusual symptoms or your condition deteriorates call your G.P. immediately.
